Our Roots

Founded by Dr. Steven Wegner, MD, JD — a nationally recognized leader in managed care — MCS builds on decades of work through Community Care of North Carolina (CCNC), where Dr. Wegner led efforts to deliver population health management for millions of Medicaid and Medicare patients.

What We Do

MCS delivers non-face-to-face, team-based care coordination for primary care practices. Using standardized protocols for 35 chronic conditions, embedded within the Smartlink Health platform, our model enables scalable implementation, rapid patient support, and measurable results.

Growth & Partnership

In October 2021, MCS was acquired by Cary Medical Management (CMM), expanding our ability to bring proven care management protocols and technology to independent practices nationwide. Together, we’re committed to delivering high-quality, efficient care management that benefits both patients and providers.
