Welcome to Chronic Care Management (CCM)

The goal of CCM is to help you manage your health conditions and prevent them from getting worse. It's like having a team of cheerleaders on your side, keeping you motivated and on track!

Here's How the CCM Program Works

  • Your health, your goals: We create a personalized care plan just for you, with clear goals and steps to reach them. Think of it as your roadmap to optimal health .
  • Always be in touch: Need help? Got questions? Our care coordinators are just a phone call away, 24/7, even on holidays and weekends.
  • Stay comfy, stay informed: We'll connect with you regularly at home, through phone calls or video chats, to offer support, answer questions, and provide helpful resources.
  • Medications made easy: Managing pills can be a hassle. We'll help you understand and stick to your medication schedule, so you can stay on top of your health.
  • Talking to your other doctors: No more playing phone tag between different offices! We'll bridge the gap and make sure everyone involved in your care is on the same page.
How do I get started with CCM?

You can enroll in Medicare's CCM program if you have two or more chronic conditions. It's super easy to get started. You simply discuss the program with your doctor and let them know you want to enroll.


Once enrolled, one of our care managers will contact you to start the process. If you have any questions, please call us at 919-258-2300. We'll be happy to answer your questions, and provide any information you need to better understand CCM.

Does my insurance cover CCM?

CCM is like hiring a friendly, experienced guide to help you navigate your health journey. The fee is $62 a month, and Medicare will pay 80% of it once you meet your annual deductible. Some secondary insurances will cover the copay. Depending on your coverage, you may or may not pay anything for the service. If you have to pay the copay, it will be somewhere between $10-12 monthly after your deductible is met.


Our team will check your insurance and tell you what to expect financially before you get started in the program. If you do have a copay, it's important to recognize that CCM has the potential to save you a lot of money on out-of-pocket expenses in the longrun. Our goal is to keep you healthy and out of the ER or hospital, which can have super expensive copay bills.
