How CCM Works for You
Getting Started with CCM
If you have two or more chronic conditions, you may qualify for Medicare’s CCM program. Enrollment is simple: talk to your doctor, let them know you’re interested, and we’ll take care of the rest.

Coverage and Costs
Most patients pay little or nothing for CCM.

The standard fee is $62 per month.

Medicare covers 80% after your deductible.

Many secondary insurance plans cover the remaining amount.

If a copay applies, it’s typically only $10–$12 per month.
Importantly, CCM is designed to save you money by preventing costly ER visits or hospital stays — while giving you peace of mind and ongoing support.
